The term “D-SNP” is short for Dual Eligible Special Needs Plan. Our plan is called PacificSource Dual Care (HMO D-SNP). It is a Medicare Advantage plan with extra benefits at no additional cost to people who qualify for Medicare and full Medicaid. It combines the benefits of Original Medicare, Part D prescription drug coverage, and Medicaid benefits.
