What is PACE?
Coverage that cares. Coordinated by one team.
PACE (Program of All-Inclusive Care for the Elderly) is a federally recognized care model for older adults who have complex medical issues, need nursing home–level care, and want to remain at home.*
For your clients, their families, and caregivers, it means one integrated team of physicians, nurses, social workers, and therapists to manage medical care, prescriptions, transportation, adult day services, specialist care (such as cardiology, pulmonology, and orthopedics), and more. And it all happens at our PACE Center in Springfield, Oregon. In addition to the comprehensive care, qualifying participants can take advantage of meals, social activities, and even haircuts and styling. PACE leaves no gaps—nothing for your clients and patients to navigate alone.

*As defined by the state of Oregon: Assessed at Service Priority Level (SPL) 1-13 as needing help meeting daily health or personal care needs (services not usually covered by traditional health insurance or Medicare). Products provided by PacificSource Health Plans, PacificSource Community Solutions, PacificSource Community Health Plans, or PacificSource Administrators, Inc. PacificSource Community Health Plans is an HMO, HMO-DSNP, and PPO plan with a Medicare contract and a contract with Oregon Health Plan (Medicaid). Enrollment in PacificSource Medicare depends on contract renewal.
†PacificSource PACE serves eligible seniors throughout Lane County except for these zip codes: 97498, 97439, 97493, 97390, 97480, 97430, 97324, 97490, 97461.
Participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services), from PacificSource PACE or from an entity authorized by PacificSource PACE. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
